Eligibility Information

1. Athlete Physical (mandatory completion before participation)

• Obtain physical form from any of the coaches or online

• Click here to get Physical form online.

• The physical must be completed by a physician, signed by the parent/legal guardian and turned in to Coach Williams. 

2. Rank One pre-participation forms (mandatory completion before participation)

• Emergency Card

• UIL Participation form (These two forms will be completed clicking the following icon.)

        

 

• Create an account or login and begin submitting information electronically. You don't have to print this document. Coaches have direct and immediate access to completed athlete paperwork. 

 3. Coaches will check grades WEEKLY.

• Students should pass every class with a minimum score of 70.
